REQUIRED form for HMS Volunteering/Shadowing

Anyone planning to volunteer or shadow at HMS School for Children with Cerebral Palsy must:
1) complete the form found on the Jeff Cares for Kids PULSE site under Documents

2) provide a copy of a TB skin test (PPD) that has been administered within the past 3 months 

Please bring these items with you if you are coming to shadow/volunteer at HMS soon, or give them to a JC4K board member so that we may hand them off to the appropriate personnel at HMS.
